The story begins with a couple, Federico and the pregnant Vanesa, stranded by a storm. They find lodging at a remote provincial hotel, with free breakfast included. Vanesa senses danger and wants to leave, but Federico insists they stay, as no other shelter is nearby.
The hotel is run by an older couple: keeper Remedios and groundskeeper Jacinto. They have teenage twins, Francisco and Lucila. Remedios is pleased to learn Vanesa is four months pregnant. The twins spend time with another guest couple, Esteban and Teresa Rivero, playing a Ouija-style game to contact their father. Jacinto catches Esteban looking through his gardening tools. Esteban becomes horrified and leaves, convinced something sinister occurred there. As the game continues, Esteban, Teresa, and the twins have visions suggesting Jacinto killed the twins’ father and buried him on the property. Jacinto tells Remedios they should reveal the truth, which angers and frightens her.
The film then flashes back fifteen years. On another stormy night, a man and his pregnant wife arrived at the hotel. Remedios, a deeply disturbed religious woman, had been praying for children. The wife went into labor, and Remedios helped deliver the twins. She then smothered the mother with a pillow, believing the newborns were God’s answer to her prayers. When the husband saw his wife dead, he attacked Remedios. Jacinto killed him with a hammer.
Fearing Jacinto would confess to the twins, Remedios kills him and stages a suicide by hanging. The twins find his body, and Remedios feigns grief, blaming them. Once they leave, she stops crying and speaks to Jacinto’s corpse. The twins eventually learn the truth about their parents’ deaths and how they came to live at the hotel. When they confront Remedios, she kills them. The film then returns to its opening scene, with Federico and Vanesa arriving. Remedios greets them warmly, offering free breakfast.
